Our vision is to provide all our patients with access to skilled and compassionate end-of-life care at the time and place of their choosing. We focus everything we do in the care of the patient and their family directly through our care services as well as our education and support programs.
We care for people in their own homes. Our assistance is centered on the unique physical, emotional, and practical needs of the individual. Our tailored care programs respect the personal and cultural wishes of our patients and their families.
WHO ARE THE PROFESSIONALS WE SEND TO YOUR HOME?
- Registered Nurses (RNs) provide individual care and case management for medically prescribed treatment.
- Licensed Vocational Nurses (LVNs), working under physicians’ orders and RN supervision, administer medication and treatment and assist the patient with returning to independent activities of daily living.
- Certified Home Health Aides provide personal care and aid in the overall improvement of the patient’s quality of life.
